I think I've topped it just shy of 29K. Bump up the gears before you start (as you say), move to a certain position then try not to move if at all possible as every move reduces the total amount... it also helps to do very slight moves rather than very big ones. That said I think anything above 25K is a very good total. 
I've had 28k but I don't go for the score on this mode, I start in 5th but when the first car comes down the middle I quickly drop to 3rd then play the rest out at that speed. In 3rd there is a pretty good chance to pick up the extra ball should it come along, any faster then it seems impossible to reach it.
Yes the letters EB come along but not very often.
Check if the machine is in tournament mode as it gets removed if it is.
